Job Summary

We are seeking a highly skilled and compassionate Qualified Intellectual Disabilities Professional to join our team at Community Options, Inc. in Brooklyn, NY. As a key member of our staff, you will play a vital role in providing person-centered care to individuals with intellectual and developmental disabilities.

About the Role
  • Develop and implement individual service plans that meet the needs of people with disabilities and state guidelines.
  • Coordinate admission and discharge activities, ensuring seamless transitions for participants.
  • Act as a liaison between area programs and clinical consultants to address any issues that may arise.
  • Provide support and supervision to staff members, focusing on compliance and clinical best practices.
  • Maintain accurate records, including monthly goal progress notes and billing justification forms.
Requirements
  • Bachelor's degree and two years' experience in implementing community-based support services for people with disabilities.
  • Valid driver's license and good driving record.
  • QIDP qualification is required.
  • Excellent organizational skills and ability to prioritize tasks under time constraints.
  • Effective communication skills, enabling you to work collaboratively with program participants, agencies, and community members.
