Key Responsibilities
  • Develop and Implement Patient Engagement Strategies: Create and execute personalized patient engagement plans to improve health outcomes and patient satisfaction.
  • Provide Health Coaching and Education: Offer one-on-one health coaching and education to patients to help them set and achieve their health goals.
  • Monitor and Evaluate Patient Progress: Regularly monitor patient progress and adjust engagement strategies as needed to ensure optimal health outcomes.
  • Collaborate with Care Team Members: Work closely with other care team members to ensure seamless communication and coordination of patient care.
  • Maintain Accurate Records: Maintain accurate and up-to-date records of patient interactions, progress, and outcomes.
Requirements
  • Medical Assistant Certificate: Possess a valid Medical Assistant certificate.
  • 2+ Years of Medical Assistant Experience: Have at least 2 years of experience as a Medical Assistant.
  • Bilingual (English and Spanish): Be fluent in both English and Spanish.
  • Experience with EHRs/EMRs: Have experience working with Electronic Health Records (EHRs) and Electronic Medical Records (EMRs).
  • Excellent Communication and Documentation Skills: Possess excellent verbal and written communication skills, as well as strong documentation skills.
  • Ability to Build Rapport with Patients: Be able to build strong relationships with patients and their families.
  • Compassionate and Caring Demeanor: Demonstrate a compassionate and caring demeanor when interacting with patients.
  • Intermediate to Advanced Computer Skills: Possess intermediate to advanced computer skills, including the ability to navigate multiple systems.
  • Reliable Internet Connection: Have a reliable internet connection to ensure seamless communication and data exchange.
