Job Summary

Vigilance Health is seeking a highly motivated and compassionate Care Manager to join our team. As a Care Manager, you will play a critical role in providing exceptional patient care and support to our clients.

Key Responsibilities
  • Provide comprehensive patient engagement and activation through consistent, timely, and effective communication.
  • Develop and implement personalized care plans to drive better clinical outcomes and patient satisfaction.
  • Collaborate with patients to set SMART goals and provide ongoing support and motivation to achieve their health objectives.
  • Maintain accurate and up-to-date patient records, ensuring seamless communication and coordination with the care team.
  • Identify and address potential care gaps, ensuring timely interventions and improved patient outcomes.
Requirements
  • Valid Vocational Nurse (LVN/LPN) License
  • 2+ years of experience as a Licensed Vocational Nurse (LVN/LPN)
  • Bilingual (Fluent in English and Spanish)
  • Experience working with EHRs/EMRs
  • Strong knowledge and background of chronic conditions
  • Ability to build rapport with patients and provide empathetic support
  • Excellent documentation and verbal communication skills
  • Intermediate to advanced computer skills with the ability to navigate multiple systems
  • Reliable internet connection
