Job Summary

The Social Services Assistant will play a critical role in ensuring the emotional and social needs of our residents are met. This includes conducting social services evaluations, providing support with financial planning, and coordinating discharge planning. The ideal candidate will be compassionate, dedicated, and able to work effectively with residents, families, and facility staff.

Responsibilities
  • Conduct social services evaluations and identify resident problems/needs
  • Provide support with financial planning and coordinate discharge planning
  • Meet with residents and families to review policies and provide information
  • Assist with resident transfers and changes in level assignment
  • Act in compliance with all corporate, state, federal, and other regulatory standards
Requirements
  • High school diploma or equivalent required
  • One year of experience in long-term care preferred
  • Ability to assess resident needs and coordinate discharge planning
  • Qualified, compassionate, and dedicated to a job well done
