Job Summary

The Licensed Clinical Social Worker will work collaboratively with a multidisciplinary team to provide high-quality care to patients. This role will involve performing diagnostic evaluations, reviewing medical history, and providing therapy for individuals. The successful candidate will also be responsible for communicating with other providers and school personnel, as necessary, to provide crisis intervention.

Key Responsibilities
  • Perform diagnostic evaluations and provide therapy for individuals
  • Communicate with other providers and school personnel to provide crisis intervention
  • Complete accurate documentation of patient encounters in Electronic Health Record (EHR) within 48 hours
  • Review and address tasks daily, entering charges into system daily for billing
  • Educate patients on community resources and how to resume life within limits of disability
  • Collaborate with fellow LCSWs, Clinical Director of Behavioral Health, and Associate Medical Director of Behavioral Health to provide quality counseling and diagnostic services
  • Provide counseling, diagnostic, and case management services to children, teens, and adults within the LCSW's age range scope of competence
  • Ensure confidentiality and HIPAA compliance at all times
  • Keep other care team members informed when situations occur that disrupt patient flow
  • Contribute to community give back initiatives and be an advocate for Premier's mission
  • Adhere to patient care needs and core values of Premier Community Healthcare to provide best-in-class patient experience
  • Work towards attaining department goals and drive Quality Improvement / Quality Assurance (QI/QA)
Requirements
  • Master of Social Work (MSW) Degree from an accredited program
  • Current SW license in Florida
  • Current Basic Life Support (BLS) certification
  • Two years' experience in resource coordination and counseling in community healthcare preferred
  • Obtain and maintain Epic certification/annual compliance training throughout employment
