About the Role

We are seeking a skilled and compassionate Registered Nurse to join our Home Health team at Providence Health & Services. As a Home Health RN, you will provide high-quality patient care in the comfort of their own homes, working collaboratively with an interdisciplinary team to deliver whole-person care.

Responsibilities
  • Provide skilled nursing care to patients in their homes, including wound care, medication management, and education.
  • Conduct thorough assessments of patients' physical and emotional needs, developing personalized care plans to meet their unique requirements.
  • Collaborate with other healthcare professionals, including physicians, therapists, and social workers, to ensure comprehensive care.
  • Communicate effectively with patients, families, and caregivers to ensure seamless care transitions and address any concerns or questions.
  • Document patient progress, including assessments, care plans, and outcomes, in accordance with regulatory requirements.
Requirements
  • Current California Registered Nurse License.
  • National Provider BLS - American Heart Association certification.
  • 1 year of professional nursing experience, preferably in home health or a related field.
Preferred Qualifications
  • Experience with IV and central lines, wound care, and negative pressure devices.
  • Home health field experience, with a focus on restorative care and longitudinal care.
