About the Role

As a Registered Nurse in our Home Health team, you will have the opportunity to provide high-quality, patient-centered care to individuals in the comfort of their own homes. Our team is dedicated to delivering exceptional care and support to our patients, and we are seeking a skilled and compassionate RN to join our team.

Responsibilities

  • Provide comprehensive nursing care to patients in their homes, including assessments, medication management, and wound care.
  • Develop and implement individualized care plans that meet the unique needs of each patient.
  • Collaborate with our interdisciplinary team to ensure seamless care coordination and communication.
  • Conduct regular patient assessments and evaluations to ensure optimal care and outcomes.
  • Document patient information and progress in a timely and accurate manner.

Requirements

  • Current California Registered Nurse License.
  • 1 year of professional nursing experience, preferably in home health or a related field.
  • Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
  • Ability to work independently and as part of a team, with a focus on delivering high-quality patient care.
  • Basic Life Support (BLS) certification.
