Job Title: RN - Home Health

We are seeking a skilled and compassionate Registered Nurse to join our Home Health team. As a Home Health RN, you will provide high-quality patient care in the comfort of their own homes, working collaboratively with an interdisciplinary team to deliver whole-person care.

About the Role:
  • Provide skilled nursing care to patients in their homes, focusing on their unique needs and goals.
  • Collaborate with the care team to develop and implement individualized care plans.
  • Conduct thorough assessments, identify patient needs, and develop effective care strategies.
  • Administer medications, perform treatments, and provide education to patients and their families.
  • Document patient progress, communicate with the care team, and participate in quality improvement initiatives.
Requirements:
  • Current California Registered Nurse License.
  • 1 year of professional nursing experience, preferably in home health or a related field.
  • Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and the care team.
  • Ability to work independently and as part of a team, with a focus on delivering high-quality patient care.
