About the Job

We are seeking a skilled and compassionate Registered Nurse to join our Home Health team at Providence St. Joseph Health Seacrest Home Health. As a Home Health RN, you will provide high-quality patient care in the comfort of their own homes, working collaboratively with an interdisciplinary team to deliver whole-person care.

Responsibilities
  • Provide skilled nursing care to patients in their homes, focusing on their unique needs and goals.
  • Conduct thorough assessments, develop and implement individualized care plans, and coordinate with other healthcare professionals to ensure seamless care transitions.
  • Collaborate with patients, families, and caregivers to educate them on disease management, medication adherence, and self-care techniques.
  • Document patient progress, identify areas for improvement, and adjust care plans as needed.
  • Participate in interdisciplinary team meetings to discuss patient care, share best practices, and address any challenges or concerns.
Requirements
  • Current California Registered Nurse License.
  • National Provider BLS - American Heart Association certification.
  • 1 year of professional nursing experience, preferably in home health or a related field.
  • Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
  • Ability to work independently, prioritize tasks, and manage time efficiently in a fast-paced environment.
Preferred Qualifications
  • Experience with wound care, IV therapy, and central lines.
  • Knowledge of home health regulations, policies, and procedures.
  • Fluency in Spanish or other languages spoken by the patient population.
