Job Title: RN - Home Health

Providence Health & Services is seeking a skilled Registered Nurse to join our Home Health team. As a Home Health RN, you will provide high-quality patient care in the comfort of their own homes.

About the Role:
  • Provide comprehensive nursing care to patients in their homes, including assessment, planning, implementation, and evaluation of care.
  • Collaborate with an interdisciplinary team to develop and implement individualized care plans.
  • Conduct regular home visits to assess patient needs and provide education on self-care and disease management.
  • Communicate effectively with patients, families, and healthcare providers to ensure seamless care coordination.
  • Participate in quality improvement initiatives to enhance patient outcomes and satisfaction.
Requirements:
  • Current California Registered Nurse License
  • 1 year of professional nursing experience, preferably in home health or a related field
  • Strong communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Basic computer skills and familiarity with electronic medical records
