Position Overview:

We are seeking a full-time Patient Navigator for our Manhattan location. The ideal candidate should possess a personable demeanor, excel in engaging with a diverse clientele, and communicate effectively with both medical professionals and support staff. Attention to detail is crucial in this role.

Key Responsibilities:

  • Deliver direct support services to individuals living with HIV/AIDS under the guidance of the Care Coordinator.
  • Implement tasks essential for executing medical and support service plans, which may include accompanying patients to appointments, providing coaching, conducting health education sessions, and coordinating logistics for plan adherence.
  • Conduct field-based tasks such as observing and documenting patient self-administration of antiretroviral therapy (ARVs), assessing and reporting any side effects, and providing support in various convenient locations.
  • Maintain accurate client-related records and documentation in accordance with the standards of Apicha's Client Services Department.
  • Collaborate with the care team, which may include doctors, nurses, and other staff, to ensure optimal health outcomes for patients.
  • Represent the organization at various events to promote our services.
  • Perform additional duties as assigned.

Qualifications:

  • Associate's degree in human services or social work, with a minimum of 12 months of experience in patient navigation or a related field.
  • Consideration may be given to candidates with peer worker certification from the NYS AIDS Institute or case management certification from CUCS, or equivalent training.
  • Strong socio-cultural identification with the target population or relevant experience is required.
  • Fluency in one or more Asian languages is preferred.
  • Excellent written and verbal communication skills in English are mandatory.
  • Proof of immunization or immunity to specific communicable diseases and tuberculosis testing is required, in compliance with health regulations.
