Key Responsibilities:
- Evaluate patients' medical or physical condition and assess patient needs in collaboration with other healthcare professionals.
- Advocate for patients to resolve crises and provide support during challenging situations.
- Collaborate with clinic staff to develop treatment plans and ensure seamless care coordination.
- Investigate child abuse and neglect cases, taking authorized protective action when necessary.
- Intervene in crisis situations to assist patients, families, and caregivers.
- Refer clients to community or social service programs, educational or work programs, and coordinate with care managers and I/DD organizations.
- Assist with Premier HealthCare initiatives, including PCMH, and participate in bi-weekly clinical operations meetings and monthly QA/QI meetings.
- Complete various forms on behalf of patients and may supervise MSW students and other interns/volunteers with appropriate credentials.
Requirements:
- Masters of Social Work from an institution accredited by the Council on Social Work Education (CSWE) or its equivalent.
- Knowledge of medical terminology and understanding of the roles of the healthcare team.
- Exceptional interpersonal skills, written and verbal communication skills, and strong crisis intervention skills.
- LMSW or LCSW preferred, with experience in community facilities and resources for discharge planning.
