Care Coordination at Saint Luke's Health System
The Care Coordinator plays a vital role in ensuring quality patient outcomes and efficient care delivery at Saint Luke's Health System. This position is responsible for developing and implementing discharge plans, mitigating obstacles to care progression, and ensuring clinical readiness for discharge.
As a key member of the multi-disciplinary team, the Care Coordinator works to optimize resource utilization, streamline care transitions, and enhance patient satisfaction. Key Responsibilities:

Requirements:
