Essential Skills:
- Detail-oriented with the ability to manage a demanding workload.
- Proficient in data entry and familiar with medical terminology.
Qualifications:
- High School diploma or GED required.
- Minimum of 2 years of relevant experience in a care management or administrative role.
Additional Responsibilities:
- Provide outreach to members via phone to assist with care plans, resource navigation, and address any questions or concerns.
- Support members in connecting with health plan and community resources to enhance their care experience.
- Serve as the first point of contact for various member and provider inquiries, explaining care procedures and protocols.
- Assist with member onboarding and manage day-to-day administrative tasks, including correspondence and educational materials.
- Ensure non-clinical member records are maintained in accordance with regulatory standards.
- Address Social Determinants of Health (SDOH) needs through appropriate referrals.
