Key Responsibilities- Conduct thorough assessments to determine physical, psychosocial, and environmental needs.
- Manage a primary caseload, including planning, implementing, and evaluating individualized care plans.
- Collaborate with the Interdisciplinary Team to address patient and family needs holistically.
- Reevaluate patient care during each visit and adapt plans to meet evolving needs.
- Communicate significant health changes to the appropriate parties promptly.
- Teach behavioral health philosophies, pain management, and care principles in various settings.
- Supervise and document the care provided.
- A current, unrestricted state RN license.
- 1+ years of clinical nursing experience.
- A dependable vehicle, valid driver's license, and current auto insurance.
- Willingness to travel up to 50% within the branch/office coverage area.
- An Associates or Bachelor's Degree in Nursing (preferred).
- Previous experience in Home Health, Hospice, or with Homecare HomeBase (preferred).
