Medical Care Coordinator

We are seeking a dedicated Licensed Practical Nurse to join our team in Western Massachusetts. As a key member of our healthcare team, you will provide essential nursing services for our residents, ensuring their health needs are met and regulatory requirements are complied with.
Responsibilities- You will respond to inquiries from Acquired Brain Injury (ABI) residences and administration regarding participant health needs, providing expert guidance and support.
- Your role involves coordinating medical treatment, including triage and emergency interventions, while maintaining open communication with healthcare providers.
- You will conduct routine and emergency visits to residences for assessments, education, and staff support, promoting a safe and healthy environment.
- Medication audits and reviews of medical documentation, including medication administration and lab results, will be an integral part of your responsibilities.
- You will develop and implement training programs for staff in collaboration with the Nurse Coordinator, enhancing our team's knowledge and skills.
- Transporting or accompanying participants to medical appointments as necessary is also an important aspect of this role.
- You will participate in clinical assessments and provide recommendations, utilizing your expertise to inform our care delivery.
- Engagement with families of participants to discuss health care matters is crucial for providing holistic care.
- Maintaining a safe work environment by identifying hazards, reporting incidents, and adhering to MHA safety policies is a top priority.
- You must hold a current and valid Massachusetts LPN license.
- A strong understanding of nursing practices and procedures is essential for success in this role.
- At least one year of nursing experience is preferred, but not required; what matters most is your ability to provide high-quality care.
- Strong self-direction, communication, and organizational skills are vital for managing multiple priorities and responsibilities.
- A valid driver's license and reliable transportation for work-related duties are also required.
The estimated salary for this role is $55,000 - $65,000 per annum, depending on experience. Our benefits package includes:
