Job Summary

Wellstar Health System has an opportunity for a Senior Care Coordinator Registered Nurse (RN) - PRN Day Shift.

Job Responsibilities
  • Transitional Care Planning: Assess transitional care needs, coordinate care across the continuum, and engage with patients and families to ensure care needs are met.
  • Clinical Care Progression: Partner with physicians and care teams to drive optimal and efficient treatment plans to streamline progression of care while in the hospital, as well as, planning for post-discharge care.
  • Patient and Family Engagement: Serve as an expert resource and consultant to the care team, providing guidance on care progression and assisting in planning to effectively meet patient needs, manage length of stay, and promote efficient utilization of resources.
  • Collaboration and Communication: Collaborate effectively with the utilization review nurse, patient's physicians, and the interdisciplinary care team to provide a comprehensive assessment of the patient's medical care needs, psychosocial needs, any social determinants of health needs, goals/outcome attainment, and continued care needs.
  • Discharge Planning: Ensure that the patient is progressing towards their discharge goal and assist to alleviate barriers, seeking consultation from appropriate disciplines/departments as required to proactively identify and resolve delays to expedite care and facilitate discharge.
  • Mentorship and Professional Development: Participate in orientation and precepting of new employee hires (as needed), mentoring new hires in clinical progression/case reviews and efficient transitional/discharge planning, and complete all initial and ongoing professional competency assessment, required mandatory education, population-specific education.
Requirements
  • Education: Bachelor's degree in Nursing.
  • Licensure: Registered Nurse (RN) licensure in the state of practice.
  • Certification: Basic Life Support (BLS) certification, Certified Case Manager (CCM) certification preferred.
  • Experience: Minimum 3 years of experience as a staff nurse in an acute care hospital setting, and minimum 2 years of experience as a case manager in a hospital setting or payer-based model with expertise in case management competencies.
