Job Summary

Wellstar Health System has an opportunity for a Senior Care Coordinator Registered Nurse (RN) - PRN Day Shift.

Job Responsibilities

The Senior Care Coordinator RN is responsible for assessing transitional care needs, coordinating care across the continuum, and engaging with patients and families to ensure care needs are met. The RN requires strong clinical expertise to partner with physicians and care teams to drive optimal and efficient treatment plans to streamline progression of care while in the hospital, as well as, planning for post-discharge care.

Key Functions
  • Transitional Care Planning: Develop and implement transitional care plans to ensure seamless transitions between care settings.
  • Clinical Care Progression: Collaborate with physicians and care teams to facilitate communication regarding patient care progression to ensure timely and efficient delivery of care.
  • Psychosocial and Functional Status Assessment: Assess patients' psychosocial and functional status to identify potential barriers to care and develop strategies to address them.
  • Interdisciplinary Rounds: Participate in interdisciplinary rounds to discuss patient care plans and ensure consensus among care team members.
  • Patient and Family Education: Educate patients and families on their care plans and provide resources to support their needs.
Collaboration and Communication

The Senior Care Coordinator RN will collaborate effectively with the utilization review nurse, patient's physicians, and the interdisciplinary care team to provide a comprehensive assessment of the patient's medical care needs, psychosocial needs, any social determinants of health needs, goals/outcome attainment, and continued care needs.

Precepting and Mentoring

The RN will participate in the precepting and mentoring of new employee hires and will mentor new and less senior employees in addressing challenging situations in assisting patients and families through the continuum of care.

Professional Development and Initiative

The Senior Care Coordinator RN will complete all initial and ongoing professional competency assessments, required mandatory education, population-specific education, and support department-based goals that contribute to the success of the organization.

Requirements
  • Education: Bachelor's degree in Nursing
  • License: Registered Nurse (RN) with a single state or multi-state compact license
  • Certification: Basic Life Support (BLS) certification or BLS Instructor certification; Certified Case Manager (CCM) or Accredited Case Manager (ACM) certification preferred
  • Experience: Minimum 3 years of experience as a staff nurse in an acute care hospital setting and minimum 2 years of experience as a case manager in a hospital setting or payer-based model with expertise in case management competencies
