Job Summary

As a Case Manager at Roper St. Francis Healthcare, you will play a vital role in coordinating patient care to achieve positive clinical and financial outcomes. Working collaboratively with interdisciplinary staff, you will facilitate the delivery of healthcare services and promote a collaborative care delivery model.

Key Responsibilities
  • Perform day-to-day case management and discharge planning services for patients with complex psychosocial, financial, and legal discharge planning needs.
  • Advocate and facilitate patient/family/caregiver-centric plans of care based on patient needs, preferences, and available resources.
  • Collaborate with multi-disciplinary teams to execute responsibilities, including post-acute referrals/discharge planning, crisis intervention, counseling, abuse/neglect, guardianship, and psychosocial assessment.
  • Provide accurate, timely, and appropriate documentation of all social work interventions in the medical record.
  • Update the discharge plan to ensure it is consistent and up-to-date based on patient discharge needs, appropriate post-acute level of care, patient/family/caregiver preferences, and covered services.
Requirements
  • Master's Degree in Social Work (MSW) or Bachelor's Degree in Nursing (RN) from an accredited college/university.
  • At least one year of clinical experience in case management, social work, and/or discharge planning.
  • Current licensure by the State Board of Social Work Examiners for S.C. (MSW) or registered nurse licensure in the state of South Carolina (RN).
  • Knowledge of community resources, government and commercial payer programs, benefits, and eligibility, as well as post-acute service regulatory requirements.
