About the Role

As a Home Health Nurse with Amedisys, you will have the opportunity to make a meaningful impact on the lives of patients and their families by providing much-needed care in the comfort of their own homes.

Responsibilities

  • Coordinate and oversee all aspects of patients' home care, ensuring a seamless and effective experience.
  • Collaborate with patient care teams to develop and implement personalized care plans that meet the unique needs of each patient.
  • Provide education and support to families on patient condition, needs, and care, empowering them to take an active role in their loved one's recovery.
  • Perform head-to-toe physical assessments, check vital signs, and manage medications to ensure the highest level of care.
  • Administer injections, IV therapy, or tube feedings as needed, and maintain catheters to ensure patient comfort and safety.
  • Complete documentation in a timely and accurate manner, ensuring that all necessary information is recorded and communicated to the clinical manager, physician, and care team.
  • Supervise LPNs and HHAs, providing guidance and support to ensure that patients receive the highest level of care.
  • Regularly communicate patient progress to the clinical manager, physician, and care team, ensuring that all stakeholders are informed and up-to-date.

Requirements
  • One year of clinical experience as a registered nurse (RN) is required, with a minimum of six months home health experience.
  • Current RN license, specific to the state(s) you are assigned to work, is mandatory.
  • CPR certification and a valid driver's license are also required.
  • Able to assess patient status and identify requirements relative to age-specific needs, with a strong understanding of general use and functions of the computer.
