We are seeking a highly motivated and compassionate Bachelors Level Social Worker to join our team at VitalCore Health Strategies in Norfolk, MA.

Job Summary:

The successful candidate will work as part of a multidisciplinary team to provide crisis stabilization, complete intakes and on-going evaluations, and provide consultation to in-house and community services.

Key Responsibilities:
  • Provide crisis stabilization and complete intakes and on-going evaluations
  • Participate in monthly behavioral health calls and work collaboratively with the medical team
  • Perform risk assessment of inmates and coordinate care with interdisciplinary team
  • Assist in planning and implementing the goals and objectives of programs and projects
  • Provide timely and excellent documentation skills
Requirements:
  • Bachelor's in Social Work
  • Preferred two-year employment in a social work capacity in a public or private agency or equivalent
  • Must receive a satisfactory background investigation report
