About the Role

The Department of Health and Human Services at the Norfolk Regional Center is seeking a compassionate and experienced Social Worker II to join our team. As a key member of our interdisciplinary team, you will provide basic social work services and discharge planning to adult patients with mental illness, including those who have sexually offended.

Key Responsibilities
  • Provide social work input for treatment planning and complete weekly and monthly progress notes.
  • Collect and analyze social history data to assess educational, vocational, and social needs of clients, and determine eligibility for services.
  • Serve as an advocate for clients, coordinating referrals to appropriate service agencies.
  • Negotiate provider contracts and evaluate services such as child care, transportation, and homemaker aides.
  • Participate in interdisciplinary team meetings to plan and implement client care programs.
  • Involve clients' family/guardians in treatment planning, program implementation, and referrals.
  • Conduct discharge interviews and develop client discharge plans by completing referrals to aftercare agencies and ensuring timely financial and placement arrangements.
  • Lead individual and group counseling sessions and social skills programs to support client transitions to nursing homes and residential settings.
  • Maintain detailed documentation of client social work activities and keep accurate records.
Requirements and Qualifications

We are looking for a candidate with a Bachelor's degree in Social Work and certification in Social Work, or a Bachelor of Arts degree in a related field with certification in social work by grandfathering and two years of experience in a residential setting, public welfare, or community service work. Certification as a Social Worker by the Nebraska Department of Health is required.
