Care Transition Support Specialist

Job Summary
The Care Transition Administrative Assistant plays a vital role in facilitating post-discharge plans in collaboration with Care Coordination RNs and/or Social Workers in the hospital setting. This position requires strong organizational and communication skills to prepare documents and chart copies/personal health records for submission to the Administrative team.
Key Responsibilities
- Collaborate with Care Coordination RNs and/or Social Workers to develop and implement post-discharge plans.
- Prepare and submit documents, chart copies, and personal health records to the Administrative team.
- Ensure seamless communication and coordination with healthcare teams to achieve optimal patient outcomes.

- Strong organizational and communication skills.
