Nurse Care Specialist

We are seeking a skilled Registered Nurse (RN) to join our team at IntelyCare. As a Nurse Care Specialist, you will provide expert nursing care to patients in post-acute care facilities, working closely with residents and medical staff to develop care plans and ensure the highest level of care.
Responsibilities- Provide expert nursing care to patients in post-acute care facilities
- Develop and implement individualized care plans in collaboration with residents and medical staff
- Monitor and track patient outcomes, making adjustments to care plans as needed
- Work with residents and family members to ensure informed decision-making and optimal care
- Collaborate with medical staff to ensure accurate and timely documentation of patient histories and interventions
- Provide training and supervision to nursing and support staff
- Participate in incident investigations and quality improvement initiatives
- Current New Jersey state license to practice as a Registered Nurse (RN)
- Successful completion of an approved nursing program
- At least 1 year of experience as a Registered Nurse or in a related role
- Advanced organizational and communication skills
