Position: Site Lead Registered Nurse (SLRN)

Overview:
The Site Lead Registered Nurse (SLRN) plays a pivotal role in overseeing and directing nursing staff to ensure high-quality patient care delivery. This position requires collaboration with healthcare professionals, including nurses and physicians, to manage a designated patient caseload effectively.

Key Responsibilities:
+ Conduct thorough assessments and data collection, adjusting patient care strategies based on ongoing evaluations of patients' physiological, psychological, and cognitive conditions in alignment with established care standards.
+ Execute both in-person and telephonic triage duties.
+ Maintain comprehensive and precise documentation of patient medical records, including vital signs and skin integrity assessments.
+ Develop individualized, goal-oriented care plans that prioritize patient needs and involve patient participation.
+ Ensure comprehensive patient care by adhering to a multi-disciplinary care plan, implementing interventions skillfully and consistently, and ensuring appropriate follow-up.
+ Apply knowledge of medication administration, including dosage calculations and patient rights, while obtaining necessary consents for treatments.
+ Collaborate with the Nurse Manager in recruitment efforts and assist in the orientation, training, and retention of nursing staff, ensuring their competency.
+ Evaluate the performance of nursing assistants and licensed practical nurses in conjunction with healthcare providers.
+ Fulfill additional responsibilities as required by the leadership team.

Qualifications:
+ Must be a graduate of an accredited nursing program with a valid Registered Nurse license in Connecticut. CPR certification is mandatory.
+ A minimum of one year of experience in a long-term care or rehabilitation setting is required; clinic experience is preferred.
+ Strong patient management skills are essential.
+ Must demonstrate motivation and the ability to work under supervision.
+ Proficiency in computer skills is necessary, with the ability to learn electronic medical records systems.
+ Familiarity with phone triage, referral processes, and medication refill protocols is important.
+ Knowledge of CLIA waived lab testing and basic diagnostic tools is required.
+ Excellent leadership and management skills are crucial.
+ Bilingual candidates are preferred but not mandatory.
