Job Summary

Privia Health is seeking a highly skilled RN Care Coordinator to join our team. As a RN Care Coordinator, you will play a critical role in assisting patients to reach optimal function and health in community-based settings.

Key Responsibilities
  • Telephonic and In-Person Patient Engagement: Contact patients to provide care management activities, including referrals to organizations that support identified health care needs.
  • Care Coordination: Collaborate with health team members to ensure safe, high-quality, and cost-effective care.
  • Patient Education and Support: Assess patients' knowledge of current health status and support informed decision-making, while encouraging confidence in patients' abilities to regain/optimize their own health.
  • Inter-Departmental Collaboration: Collaborate with inter-departmental teams and medical practice staff to facilitate program objectives and goals.
Requirements
  • ADN or Diploma in Nursing with 5+ years of nursing experience or BSN/BAN 3+ years of experience in nursing
  • 2+ years of population health or care coordination experience
  • Active unrestricted license in CT or able to obtain (if needed)
  • Strong computer and EHR skills and expertise
  • Preferred experience with Medicare-aged patients in a value-based program
  • Preferred experience educating and engaging patients
  • Experience working in an outpatient medical office
  • Ability to travel to and from care centers to meet with patients and providers
  • Must comply with all HIPAA rules and regulations
Compensation and Benefits

The salary range for this role is $80,000 to $90,000 in base pay, exclusive of any bonuses or benefits. This role is also eligible for an annual bonus targeted at 10%.

Technical Requirements

In order to successfully work remotely, supporting our patients and providers, we require a minimum of 5 MBPS for Download Speed and 3 MBPS for the Upload Speed. This should be acquired prior to the start of your employment.
