Position Overview
The Patient Care Coordinator plays a vital role in the management of administrative tasks and discharge planning for all patients. This position is essential in ensuring a seamless transition for individuals receiving care.

Key Responsibilities
1. Develop and implement a Continuing Care Plan in collaboration with patients.
2. Conduct weekly psycho-educational sessions to support patient understanding and engagement.
3. Assist patients in completing necessary paperwork for Short Term Disability and FMLA.
4. Accurately initiate, explain, and manage the Referral and Intake Processes.
5. Ensure the collection and processing of Exit Surveys, delivering them to the appropriate staff for analysis.
6. Uphold confidentiality regulations at all times.
7. Provide a warm welcome and guidance to patients and visitors.
8. Respond promptly and professionally to all communications, including calls and mail.
9. Maintain an organized filing system for patient records.

Qualifications
The ideal candidate will possess a high level of interpersonal skills, demonstrating poise, professionalism, and diplomacy in all interactions.

Skills and Abilities
- Excellent organizational skills with the ability to prioritize tasks effectively.
- Proficient in various computer software applications, including MS Word, Excel, and PowerPoint.
- Understanding of diverse cultures and gender-specific issues, with the ability to address the needs of various groups.
- Commitment to maintaining confidentiality when interacting with patients, families, and other stakeholders.
- Strong customer relations skills, fostering supportive communication with all contacts.

Education and Experience
A Bachelor's degree in psychology, social work, or a related field is preferred. A minimum of two years of experience in a mental health or healthcare setting is required. Experience with insurance verification and billing is advantageous.
