Position Overview: Case Manager

Employment Type: Full Time (40 hours weekly)

Department: Counseling Services

Work Environment: Client Homes and Office

Key Responsibilities:

  • Manage a caseload of families within the designated area.
  • Achieve productivity benchmarks as determined by program requirements.
  • Be accessible for on-call support during client emergencies.
  • Establish and nurture trusting relationships with clients.
  • Conduct treatment planning, assessments, counseling, and case management focused on substance use issues.
  • Execute case management duties in accordance with organizational policies.
  • Collaborate with local services and provide suitable referrals.
  • Maintain accurate documentation and reporting of case activities and client progress.
  • Engage in team meetings and community outreach initiatives.
  • Comply with organizational policies and regulatory standards.

Required Qualifications:

  • Strong communication and conflict resolution abilities.
  • Capability to work effectively within a team setting.
  • Possession of a valid Ohio Driver's License with a clean driving history.
  • Successful completion of a criminal background check.
  • Educational Background: Bachelor's Degree in Human Services or a related discipline, with a minimum of two years of relevant experience (Advanced degree preferred).
  • Licensure as a Social Worker or Chemical Dependency Counselor is preferred.
  • Experience: Proven experience in delivering supportive services to at-risk families and leveraging community resources.
  • Physical Demands: Ability to travel to clients' residences as needed.
