Position Overview: Case Manager
Employment Type: Full Time (40 hours weekly)
Department: Counseling Services
Location: Kent Office and Client Residences
Key Responsibilities:
- Manage a caseload of families within the designated area.
- Achieve productivity benchmarks set by program requirements.
- Be accessible for on-call support during client emergencies.
- Establish and nurture trusting relationships with clients.
- Conduct treatment planning, assessments, counseling, and case management related to substance use issues.
- Deliver case management services in accordance with organizational policies.
- Work collaboratively with local services and provide necessary referrals.
- Document and report on case activities and client progress.
- Engage in team meetings and outreach initiatives.
- Comply with organizational policies and regulatory standards.
Required Qualifications:
- Strong communication and conflict resolution abilities.
- Capacity to work effectively within a team setting.
- Possession of a valid Ohio Driver's License with a clean driving history.
- Successful completion of a criminal background investigation.
- Educational Background: Bachelor's Degree in Human Services or a related discipline, with a minimum of two years of relevant experience (Advanced degree preferred).
- Preferred licensure as a Social Worker or Chemical Dependency Counselor.
- Experience: Background in providing support services to at-risk families and leveraging community resources.
- Physical Demands: Ability to travel to clients' residences as needed.
