Job Summary:

As a Joint Replacement Program Coordinator at Excelsior Orthopaedics Group, you will play a pivotal role in ensuring the seamless coordination of care for patients undergoing total joint replacements on an outpatient basis. This position requires close collaboration with our team of orthopedic surgeons, medical staff, and healthcare professionals to optimize patient outcomes and provide exceptional care throughout the entire joint replacement process.

Duties and Responsibilities:

  • Patient Communication and Advocacy
  • Serve as a primary point of contact for patients scheduled for outpatient total joint replacements
  • Follow processes to coordinate all aspects of the patient's care, including pre-screening processes, insurance verification, pre-operative evaluations, surgical scheduling, post-operative follow-up appointments, DME services, and therapy services
  • Guide patients to an appropriate facility if they do not meet the BSC Outpatient Total Joint Requirements
  • Provide comprehensive education to patients and their families regarding the total joint replacement procedure, including pre-operative preparation, surgical expectations, and post-operative rehabilitation plans
  • Address any questions or concerns and offer guidance throughout the treatment process
  • Educate patients regarding CareSense tools to capitalize on patient education and outcomes
  • Serve as an advocate for patients, ensuring their needs and concerns are addressed throughout their total joint replacement journey
  • Act as a liaison between patients, healthcare providers, and billing department to facilitate efficient and appropriate care

Pre- and Post-operative Preparation/Care:

  • Work closely with OTJ Coordinator, surgeons, surgical schedulers, therapy, DME, the surgical recovery suite staff, billing staff, and operating room staff to schedule outpatient total joint replacement surgeries
  • Collaborate with the healthcare team to ensure patients are adequately prepared for surgery
  • Direct patients on pre-operative evaluations, including medical clearances, imaging, lab work, and any additional testing/clearances required
  • Communicate with surgical schedulers to coordinate the distribution of pre-operative instructions to patients and assist with any required paperwork
  • Guide patient on pre-operative testing, imaging, and other necessary procedures to ensure smooth surgical processes and minimize delays
  • Coordinate post-operative care plans by collaborating with therapy and DME departments
  • Monitor CareSense responses from patients and direct to appropriate surgeon as needed

Documentation and Record Keeping:

  • Maintain accurate and up-to-date patient records, including medical histories, treatment plans, and communication logs
  • Ensure compliance with legal and regulatory requirements regarding patient privacy and confidentiality
  • Provide process improvement suggestions and solutions within current practices to continuously improve the program
  • Evolve in your role when performing supplemental responsibilities as assigned

Requirements and Qualifications:

  • Bachelor's degree in a related field or Associates degree plus 5 years related experience
  • Previous experience in orthopedic or surgical patient care coordination preferred
  • Strong interpersonal and communication skills to effectively interact with patients, healthcare providers, and support staff
  • Insurance verification experience preferred
  • Knowledge of outpatient total joint replacement procedures and protocols preferred
  • EMR experience required, MEDENT preferred
  • Computer skills required with minimum proficiency in Word, Excel, and Outlook
  • Excellent organizational and multitasking abilities to manage a high volume of patients and complex schedules
  • Attention to detail and accuracy in maintaining patient records and documentation

Salary: $80,000 - $110,000 per year
