The role of the Care Coordinator is essential in delivering holistic case management services to individuals experiencing homelessness or those recently transitioned into stable housing. Utilizing harm reduction strategies, the Care Coordinator collaborates with various community programs to ensure clients maintain their housing, engage with necessary services, and achieve stability. Please note that vaccination for COVID-19 is mandatory prior to employment commencement. The hourly compensation ranges from $30 to $34.62.

Key Responsibilities:

  • Assist individuals in moving from homelessness to permanent housing solutions
  • Perform thorough assessments and create tailored service plans
  • Deliver short-term clinical case management support
  • Employ motivational interviewing techniques and provide crisis intervention
  • Advocate for clients within legal systems and connect them to essential services
  • Support clients with applications, transportation, and seamless transitions to long-term services
  • Ensure culturally sensitive interactions and participate in case conferences
  • Engage in community meetings with service providers while adhering to all organizational policies
  • Travel to various locations as required to fulfill job responsibilities

Care Coordinator (HUMS) Specific Duties:

  • Manage a caseload and conduct needs assessment interviews
  • Assist clients in securing identification and benefits
  • Facilitate client access to resources and services through referrals and transportation
  • Maintain accurate client records and documentation within the Homeless Management Information System
  • Educate clients on essential life skills, foster relationship-building, and monitor client progress
  • Build partnerships with local housing agencies and respond to inquiries about housing resources
  • Support the H3 CalAIM team and carry out all assigned responsibilities

Qualifications:

  • Knowledge of psychiatric disorders and evidence-based intervention models
  • Ability to cultivate supportive relationships with diverse populations while practicing cultural sensitivity
  • Commitment to principles of Diversity, Equity, and Inclusion
  • Strong networking, organizational, and time management skills
  • Proficiency in crisis intervention and familiarity with computer applications
  • Capability to perform tasks in challenging environments and travel as needed

Education and Experience:

  • Bachelor's Degree in Public Health, Counseling, Social Work, or a related discipline
  • A minimum of 2 years of experience working with individuals facing mental health challenges or homelessness
  • Preferred qualifications include bilingual abilities and prior outreach and case management experience

Certificates and Licenses:

  • CPR and first aid certification required within 90 days of hire
  • A valid California driver's license, proof of insurance, and reliable transportation are necessary

Physical Requirements:

  • Ability to stand, walk, sit, handle objects, reach, climb, lift/carry, and push/pull items
  • Excellent vision is required; taste and smell are not applicable

Work Environment:
General office setting and homeless shelter
