Job Title: Certified Peer Support Specialist

At Resolute Home Care NC, we are seeking a Certified Peer Support Specialist to join our team. As a Certified Peer Support Specialist, you will play a vital role in providing coaching, mentoring, and consultation to beneficiaries to promote recovery, self-advocacy, and self-direction.

Job Summary:

The Certified Peer Support Specialist is responsible for providing essential expertise and consultation to the entire interdisciplinary healthcare team to promote a culture in which each individual's point of view and preferences are recognized, understood, respected, and integrated into treatment, rehabilitation, and community self-help activities.

Key Responsibilities:
  • Provide services based on principles of wellness self-management, personal recovery, natural supports, coping skills, self-advocacy skills, and independent living skills for housing, employment, and community inclusion.
  • Assist clients with self-determination and decision-making.
  • Teach functional skills (managing meds, finances, health care, resources, daily living, etc.).
  • Model recovery values, attitudes, beliefs, and personal action to encourage wellness.
  • Teach and promote self-advocacy and empower clients to use their legal rights.
  • Assist with crisis plan development, advanced directives, and relapse prevention support.
  • Support stable housing and enhance independent living skills.
  • Assist clients with educational and job training opportunities.
  • Develop and implement a Mental Health Wellness Recovery Action Plan.
  • Provide linkage to community resources.
  • Provide case management services.
  • Utilize personal recovery experience to instill hope and optimism.
  • Demonstrate a personal belief in recovery and supported employment best practices.
  • Engage and serve enrolled consumers, contributing to rehabilitative success.
  • Promote a culture of recovery and empowerment within and outside of the organization.
  • Ensure timely completion of documentation per Clinical Coverage Policy NC 8A and 8G.
  • Maintain client confidentiality in adherence to HIPPA regulations.
  • Accurately document all billable encounters into the EMR system within 24 hours.
  • Participate in all required training and education as mandated by clinical coverage policy.
  • Collaborate with supervisor to identify training and supervision needs.
  • Perform other duties as assigned to support quality service provision and team support.
Requirements:
  • Education/Experience: Must possess at a minimum a High School Diploma/GED.
  • NC Certified Peer Support Specialist.
  • Valid driver's license.
  • Must have North Carolina Peer Support Certification and maintain current certification.
Work Environment:
  • Work is performed primarily in the client's living environment and in the community.
  • This may involve exposure to cigarette smoke, domestic animals/pets, household pests, uncomfortable heating/cooling, and other issues related to the domestic location in which service is delivered.
Physical Demands:
  • While performing the duties of this job, the employee is regularly required to stand, walk, use hands to finger, handle, or feel objects, tools, or controls; talk and hear; and taste and smell.
  • The employee frequently reaches with hands and arms and stoops, kneels, crouches, or crawls.
  • The employee occasionally sits and climbs or balances.
  • The employee must be able to lift and/or move up to 20 pounds and occasionally lift and/or move up to 50 pounds.
  • Specific vision abilities required include close vision, distance vision, color vision, peripheral vision, depth perception, and the ability to adjust focus.
  • Must be able to drive and operate a personal vehicle safely and adhere to all applicable state and traffic laws.
