Job Overview

Position Title: Transitional Housing Case Manager

HOPE of East Central Illinois is committed to eradicating domestic violence through a comprehensive array of services aimed at supporting victims and fostering societal change. We are currently looking to fill a Full-Time Transitional Housing Case Manager position.

Role Summary:

This is an exempt, full-time role requiring 40 hours per week, with potential evening and flexible hours. The Transitional Housing Case Manager will be responsible for delivering case management and advocacy services to adult clients and their dependents enrolled in our transitional housing program. Key responsibilities include:

  1. Conducting client intakes and assessments
  2. Offering counseling, life skills assistance, advocacy, and other essential services
  3. Facilitating life skills groups and tenant meetings
  4. Providing transportation for clients
  5. Collaborating with clients to establish safety and goal plans, assisting them in achieving and updating these objectives
  6. Monitoring and evaluating client progress towards their goals
  7. Documenting all client interactions
  8. Maintaining and updating client files
  9. Implementing and overseeing agency policies to ensure client safety
  10. Participating in hotline and on-call rotations
  11. Performing additional duties as assigned
Qualifications:

Education and Certification:

  • Valid Driver's License and Auto Insurance - Required
  • Bachelor's Degree - Required
  • 40-Hour Training in Domestic Violence - Provided
  • Successful completion of a background check - Required

Experience:

  • 1-3 Years of Relevant Experience - Required
  • Experience with Domestic Violence Victims - Preferred

Skills and Abilities:

  • Ability to sit or stand for extended periods - Required
  • Capability to lift a minimum of 50 pounds - Required
  • Strong written and verbal communication skills - Required
  • Basic computer proficiency - Required
  • Ability to comprehend and interpret written information - Required
