We are seeking a highly skilled and dedicated individual to join our team as a Case Manager/QIDP. As a key member of our team, you will be responsible for providing exceptional service to individuals with developmental disabilities in Moultrie, Douglas, and Coles counties in IL.

Job Summary

The successful candidate will possess a bachelor's degree in human services and QIDP eligibility. They will be responsible for assessing individual needs, coordinating services, and advocating on behalf of our clients. The ideal candidate will have excellent communication and problem-solving skills, with the ability to work effectively in a team environment.

Key Responsibilities
  • Conduct intake/PUNS interviews and annual reviews as needed
  • Provide linkage to community services and advocate on behalf of clients
  • Coordinate specialized services for eligible Bogard individuals
  • Monitor services provided to Bogard class members and advocate for unmet needs
  • Document services delivered and maintain individual files
  • Participate in scheduled all-staff meetings and mandated training sessions
Requirements
  • Bachelor's degree in human services and QIDP eligibility
  • Excellent communication and problem-solving skills
  • Ability to work effectively in a team environment
  • Reliable transportation and a valid Illinois Driver's License
