We are seeking a dedicated Oncology Patient Care Coordinator to help us achieve our objectives.

Role Summary and Qualifications

As an Oncology Patient Care Coordinator, you will be an integral part of a multidisciplinary team, acting as an advocate and educator for cancer patients from the point of diagnosis through the conclusion of active treatment.

Your responsibilities will include ensuring patient adherence to treatment protocols. You will accomplish this by fostering relationships with patients and healthcare providers, coordinating care plans, providing education, and facilitating access to resources within the multidisciplinary care framework. You will also be accountable for managing resources to ensure high-quality outcomes for cancer patients, including collaboration across facilities.

  • You will act as a liaison between patients, families, and healthcare professionals to promote effective communication and teamwork.
  • You will advocate for patients from diagnosis through the end of their treatment.
  • You will evaluate patients' medical, social, and psychological needs and connect them with appropriate resources.
  • You will deliver essential education, outreach, and support to patients and their families, empowering them to take charge of their healthcare.
  • You will conduct follow-up discussions with patients and relay any concerns or changes to the relevant healthcare providers.

Required Qualifications:

  • A minimum of three to five years of experience in oncology nursing.
  • Experience in program development is preferred.
  • Background as a nurse navigator or patient educator in oncology is preferred.
  • Leadership and customer service experience are preferred.

Educational Requirements:

  • An Associate's Degree in Nursing is required.
  • A Bachelor's Degree in Nursing is preferred.

Licenses and Certifications:

  • Basic Cardiac Life Support (BCLS) certification is required.
  • Current licensure as a Registered Nurse (RN) is required.
  • Oncology Certified Nurse (OCN) certification is preferred or in progress.
