This role requires a strong understanding of medical necessity, patient needs, and resource utilization to promote optimal health outcomes and cost-effective care.
Key Responsibilities- Collaborate with interdisciplinary teams to develop and implement patient care plans that address medical, psychosocial, and vocational needs.
- Conduct thorough assessments of patients to identify potential barriers to care and develop strategies to overcome them.
- Coordinate with healthcare providers, payers, and community resources to ensure timely and effective care transitions.
- Monitor patient progress and adjust care plans as needed to ensure optimal health outcomes.
- Communicate effectively with patients, families, and healthcare teams to ensure clear understanding of care plans and expectations.
- Associate Degree in Nursing, required
- Bachelor's Degree in Nursing, preferred
- Case Management certification, preferred
- Current Texas licensure with State Board of Nurse Examiners as an RN
- 1+ years experience in healthcare
