Job Overview

As we continue to expand, we are looking for a
Full-time Office Licensed Vocational Nurse (LVN) or an individual with Home Health Care experience to become a valuable member of our team.

This position is not remote.

Role Responsibilities:

  • Oversee Initial Intake and Insurance Verification while providing essential clinical information to field nurses.
  • Evaluate and review clinical progress notes submitted by field staff.
  • Conduct orientations and training sessions for all field personnel.
  • Manage telephone triage for new intakes, order medical supplies, and coordinate patient care.
  • Maintain an Infection Control Log and prepare incident reports as necessary.
  • Engage in Quality Assurance and Quality Improvement initiatives, including performing quality management activities on patient charts.
  • Participate in QA/QI meetings to enhance service delivery.
  • Share After Hours Phone Triage responsibilities with existing management staff.
  • Engage in OFFICE WORK aimed at achieving ACHC certification.

You will collaborate with a dedicated team consisting of 3 existing RNs and 1 LVN.
