Job Summary:

The Catholic Health System is seeking a skilled Speech Language Pathologist to provide per diem home care services to patients with speech/language/cognitive impairments or swallowing disorders. As a member of our team, you will have the opportunity to make a meaningful difference in the lives of our patients and their families.

Responsibilities:

• Provide speech therapy services to home care patients with speech/language/cognitive impairments or swallowing disorders

• Develop and implement individualized treatment plans to meet the unique needs of each patient

• Collaborate with patients, family members, and caregivers to ensure effective communication and coordination of care

• Conduct assessments and evaluations to identify areas of need and develop strategies for improvement

• Maintain accurate and detailed records of patient progress and treatment plans

• Participate in interdisciplinary team meetings to discuss patient care and coordinate services

Requirements:

• Master's Degree in Speech Language Pathology from an accredited college or university

• Certificate of Clinical Competence in Speech Pathology granted by the American Speech and Hearing Association

• State of New York license and registration required

• One year experience as a Speech Language Pathologist strongly preferred

• Valid driver's license and reliable transportation required

Working Conditions:

• Delivers speech therapy care in home/community setting working with patient, patient's family and any other available caregivers

• May deliver care independent of any other available caregivers
