Certified Hospice Care Specialist

The Hospice Aide is a vital member of our interdisciplinary team, working under the supervision of a registered nurse to provide personalized care and support to patients in their homes. This role requires a compassionate and empathetic individual who can deliver high-quality care while promoting comfort and dignity for our patients.
Key Responsibilities- Perform personal care tasks, such as bathing, grooming, and dressing, as assigned by the patient's care plan.
- Assist with meal preparation, feeding, and light housekeeping duties to ensure a safe and comfortable living environment.
- Provide emotional support and companionship to patients and their families, promoting a sense of calm and well-being.
- Document patient observations, care provided, and any significant changes in their condition, ensuring accurate and timely reporting to the care team.
- Collaborate with the interdisciplinary team to develop and implement patient care plans, ensuring continuity and quality of care.
- Participate in quality assessment and performance improvement initiatives to enhance patient outcomes and satisfaction.
- Meet the training requirements of the State.
- At least 18 years of age.
- Ability to read and follow written instructions and document care given.
- Understands hospice philosophy and is comfortable providing specialized care to the terminally ill.
- Satisfactory references from previous employers.
- Self-directing with the ability to work with little direct supervision.
- Possess and maintains current CPR certification.
- Demonstrates tact, patience, and good personal hygiene.
- Licensed driver with automobile that is insured in accordance with organization requirements and is in good working order.
- Good oral and written communication skills.
- Good organizational skills.
Salary: $26.00 per hour
