Job Summary

We are seeking a skilled Clinical Care Coordinator to join our team. As a nurse with a passion for hospice and palliative care, you will engage with patients and families to provide education, support, and guidance throughout their care journey.

Responsibilities
  • Conduct community outreach to educate patients and families about hospice and palliative care services
  • Review consents and other documents to assist with election of hospice benefit
  • Coordinate patient discharge from hospital into hospice or palliative care programs
  • Perform start of care and initial nursing assessment
  • Work cooperatively with hospice and/or palliative care teams
Requirements
  • Graduate of accredited school of nursing with current nursing license in California
  • Certification in Hospice and Palliative Nursing encouraged
  • At least one-year experience as a professional nurse within the last 3 years
  • Excellent verbal and written communication skills
  • Spanish speaking a plus, but not required
  • Valid driver's license, and auto insurance, reliable and well-maintained transportation
