Hospice Case Manager RN

We are seeking a compassionate and skilled Hospice RN Case Manager to join our team at Centilena Hospice Care. As a key member of our interdisciplinary team, you will provide high-quality patient care and support to individuals and their families during this sensitive time.
Job Summary:The Hospice RN Case Manager will be responsible for conducting assessments, developing individualized care plans, and coordinating patient care with healthcare professionals. You will also advocate for patients and ensure they receive appropriate resources and support.
Responsibilities:- Conduct thorough assessments to identify patient needs and develop comprehensive care plans.
- Collaborate with healthcare professionals to coordinate and facilitate patient care.
- Advocate for patients and ensure they receive necessary resources and support.
- Monitor patient progress and adjust care plans as needed.
- Document all interactions and interventions in a timely and accurate manner.
- LVN degree in nursing.
- Previous experience in case management or a related role preferred.
- Knowledge of critical care, behavioral health, and ER settings.
- Strong communication and interpersonal skills.
- Ability to multitask and prioritize responsibilities.
- Excellent problem-solving and decision-making abilities.
- Proficiency in electronic medical records systems.
- Competitive salary: $40.00 - $50.00 per hour.
- Full-time schedule: 40 hours per week.
- Benefits package, including free parking, paid sick time, and paid time off.
- Hospice care setting.
- 8 hour shift, Monday to Friday, with on-call and weekend responsibilities as needed.
- LVN: 3 years (Required).
- Case management: 1 year (Required).
- LVN (Preferred).
