Job SummaryWe are seeking an experienced Registered Nurse to join our team as a Hospice Care Specialist. The successful candidate will be responsible for initiating and coordinating hospice care plans, supervising Certified Hospice and Home Health Aides (CHHAs) and Licensed Vocational Nurses (LVNs), and providing skilled nursing services to patients in their place of residence.
Responsibilities- Develop and implement individualized care plans in collaboration with the attending physician and Interdisciplinary Hospice Team.
- Provide skilled nursing services, including pain management and symptom control, to patients in their place of residence.
- Conduct regular assessments and evaluations to ensure the highest level of care.
- Collaborate with patients, families, and caregivers to provide education and support.
- Utilize electronic medical records and computer systems to document patient information and communicate with the healthcare team.
- Graduate of an accredited school of nursing.
- Current California RN license.
- BSN preferred.
- Certification in Hospice and Palliative Nursing desired and encouraged.
- Minimum of one year experience as a professional nurse within the last three years or a Baccalaureate in Nursing from a program accredited by the National League for Nursing.
- Valid driver's license, auto insurance, and reliable transportation.
- Spanish speaking a plus, but not required.
