ROLE OVERVIEW:

The Substance Use Disorder Specialist plays a crucial role in delivering street-based assessments, counseling, crisis intervention, and follow-up services for individuals experiencing homelessness. This position is part of a street-based Multidisciplinary Team (MDT) and will work closely with various community organizations that offer both inpatient and outpatient substance use treatment.

KEY RESPONSIBILITIES:
  • Conducting mobile outreach to engage with homeless individuals in their surroundings.
  • Establishing trust and providing emotional support to encourage participation in outreach services and community referrals.
  • Building partnerships with substance use treatment providers and overseeing the referral process.
  • Delivering ongoing case management focused on housing, utilizing a Harm Reduction and Housing First approach.
  • Identifying individuals with substance use disorders and developing personalized treatment plans.
  • Providing follow-up services to ensure successful referrals and advocating for clients as necessary.
  • Sharing expertise on substance use disorders with team members.
  • Maintaining accurate records in both written and electronic formats.
  • Coordinating program activities and participating in meetings as directed by supervisors.
  • Utilizing personal and agency vehicles for outreach throughout the community.
  • Embracing additional responsibilities as assigned by supervisors to further our mission.
QUALIFICATIONS:
  • Expertise in field-based services, with familiarity in the Coordinated Entry System (CES) aimed at achieving 'functional zero.'
  • Comprehensive understanding of the challenges faced by homeless individuals, including health issues, substance abuse, mental health concerns, and resources for undocumented persons.
  • Empathetic and non-judgmental approach to working with homeless populations.
  • Ability to manage mental and physical health needs while applying 'harm reduction' and 'housing first' principles.
  • Knowledge of DSM-5 substance use disorders.
  • Proficient in documentation, electronic data entry, and extensive charting.
  • Experience in handling complex cases involving substance use, mental health, and physical health issues.
  • Availability for after-hours crisis intervention and disaster response.
  • Strong crisis intervention skills in emotionally charged situations.
  • Experience working within multidisciplinary teams.
  • Effective conflict resolution abilities.
  • Proficiency in Microsoft Office, Zoom, and other web-based applications.
  • Strong prioritization skills for managing tasks and deliverables.
  • Creative problem-solving and advocacy skills.
  • Excellent communication skills, both written and verbal.
ADDITIONAL REQUIREMENTS:
  • Maintain confidentiality in accordance with HIPAA standards.
  • Flexibility to adapt to a dynamic work environment.
  • Bilingual proficiency in Spanish and English is preferred.
  • Certification in CPR/First Aid is required.
  • Ability to pass post-offer Tuberculosis (TB) clearances and a pre-employment physical exam.
  • Regular travel is required for this position, approximately 30% of the time.
  • Must possess a valid California Driver's License and maintain auto insurance.
  • Use of a personal vehicle for travel between various locations is necessary.
  • Ability to work a 9/80 work schedule.
EXPERIENCE:
  • Active CADAC, CAADE, or CAPP accreditation is required.
  • A minimum of one (1) year of experience working with homeless individuals or at least two (2) years of experience in direct health, behavioral health, or substance use counseling.
