ROLE OVERVIEW:

The Substance Use Disorder Specialist plays a critical role in delivering on-the-ground assessments, counseling, crisis intervention, and service tracking for individuals experiencing homelessness. This position is integral to a street-based Multidisciplinary Team (MDT) and involves collaboration with various community organizations that provide both inpatient and outpatient substance use treatment.

KEY RESPONSIBILITIES:
  • Conducting outreach in mobile settings to engage with homeless individuals in their own environments.
  • Establishing trust and providing emotional support to encourage participation in outreach services and community resources.
  • Building partnerships with substance use treatment providers and overseeing the referral process.
  • Delivering ongoing case management focused on housing stability through a Harm Reduction and Housing First framework.
  • Identifying individuals with substance use disorders and developing personalized treatment plans.
  • Providing follow-up services to ensure referrals are completed and advocating for clients as necessary.
  • Sharing expertise on substance use disorders with team members.
  • Maintaining accurate records in both written and electronic formats.
  • Coordinating program activities and participating in meetings as directed.
  • Utilizing personal and agency vehicles for outreach throughout the service area.
  • Taking on additional tasks as assigned to further our mission.
QUALIFICATIONS:
  • Expertise in field-based services, with knowledge of the Coordinated Entry System (CES) and its objectives.
  • Comprehensive understanding of the challenges faced by homeless individuals, including health, substance abuse, and mental health issues.
  • Empathetic and non-judgmental approach to working with vulnerable populations.
  • Ability to manage complex mental and physical health needs, adhering to harm reduction and housing-first principles.
  • Familiarity with DSM-5 substance use disorders.
  • Proficient in detailed documentation and electronic data entry.
  • Experience in handling complex cases involving substance use and mental health issues.
  • Availability for after-hours crisis response.
  • Strong crisis intervention skills in high-pressure situations.
  • Experience working within multidisciplinary teams.
  • Excellent conflict resolution abilities.
  • Proficiency in Microsoft Office Suite, Zoom, and other web-based applications.
  • Strong organizational skills and ability to prioritize tasks effectively.
  • Creative problem-solving and advocacy skills.
  • Exceptional written, verbal, and interpersonal communication skills.
ADDITIONAL REQUIREMENTS:
  • Adherence to HIPAA standards for confidentiality.
  • Flexibility to adapt to a dynamic work environment.
  • Bilingual skills in Spanish and English are preferred.
  • Certification in CPR/First Aid is required.
  • Ability to pass necessary health clearances.
  • Regular travel is required for this position.
  • Valid California Driver's License and auto insurance in good standing are mandatory.
  • Ability to work a 9/80 schedule.
EDUCATIONAL AND PROFESSIONAL BACKGROUND:
  • Active CADAC, CAADE, or CAPP accreditation is required.
  • A minimum of one year of experience working with homeless individuals or two years of experience in health, behavioral health, or substance use counseling is necessary.
