Job Summary

We are seeking a skilled Nurse Navigator to join our Maternal Fetal Medicine team at Wellstar Health System. As a key member of our team, you will play a vital role in coordinating patient care and ensuring seamless communication between patients, physicians, and healthcare providers.

Key Responsibilities
  • Collaborate with patients, physicians, and healthcare providers to develop and implement individualized care plans
  • Provide education and support to patients and their families on maternal fetal medicine topics, including pregnancy risks and treatment options
  • Assist in the development and implementation of program policies and procedures to improve patient outcomes and satisfaction
  • Work closely with the OB/GYN and pediatrics divisions to evaluate patient outcomes and identify areas for improvement
  • Stay up-to-date on the latest research and best practices in maternal fetal medicine and share knowledge with the team
Requirements
  • Strong knowledge of maternal fetal medicine and patient navigation principles
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Strong analytical and problem-solving skills
  • Ability to maintain confidentiality and handle sensitive patient information
