Job Summary

We are seeking a highly skilled Registered Nurse Care Coordinator to join our Advanced Heart Failure team at Emory Healthcare. As a key member of our multidisciplinary team, you will play a critical role in coordinating patient care and ensuring optimal outcomes for our patients with heart failure.

Key Responsibilities
  • Function as a liaison between patients, caregivers, outside agencies, and our Advanced Heart Failure team to ensure seamless care coordination.
  • Utilize electronic and telephone communication to intervene early and prevent hospitalizations.
  • Develop and implement personalized care plans to achieve optimal outcomes and wellness for patients with heart failure.
  • Conduct proactive patient outreach and care coordination to ensure timely and efficient care delivery across the continuum of care.
  • Collaborate with healthcare teams and community-based organizations to improve communication and access to services.
  • Perform patient re-assessments to determine current health status and progress toward healthcare goals and care plan completion.
  • Conduct systematic evaluation of outcomes in care coordination plans and update patient care plans as needed.
Requirements
  • Graduate of an accredited nursing school.
  • Bachelor's degree in Nursing (BSN) required.
  • Valid, active unencumbered Nursing license or temporary permit approved by the Georgia Licensing Board.
  • Three (3) years of healthcare experience required.
  • BLS Healthcare Provider certifications required.
