The Samsky Advanced Heart Failure Clinic is a leading center for heart transplant and LVAD care, serving one of the largest patient populations in the nation.

This role is critical in supporting patients through the evaluation process for heart transplant and LVAD candidacy.

As a member of our multidisciplinary team, you will work closely with cardiothoracic surgeons, pharmacists, registered nurses, social workers, and dietitians to provide comprehensive care to our patients.

The ideal candidate will have a strong background in cardiovascular care and experience working with high-risk patient populations.

This is a full-time position with a regular schedule and opportunities for professional growth and development.

Job Purpose:

The RN Evaluation Coordinator will participate in the development, implementation, and maintenance of the LVAD program and the heart transplant program.

Key responsibilities will include:

  • Coordinating and managing patient care throughout the continuum
  • Developing and implementing program policies and procedures
  • Collaborating with multidisciplinary teams to ensure seamless patient care
  • Providing education and support to patients and families
Qualifications:

To be successful in this role, you will need:

  • A graduate degree in nursing
  • At least two years of professional clinical experience
  • A current license in the State of Georgia as a Registered Nurse or NLC/eNLC Multistate License
  • BLS and ACLS certification
Preferred Qualifications:

We are looking for candidates with:

  • Cardiovascular focus experience
  • One year of experience caring for LVAD patients and heart transplant patients
  • Leadership qualities, including time management, verbal and written communication skills, listening skills, decision-making, priority setting, and work organization skills
