As a Nurse Practitioner/Physician Assistant in our Heart Failure program, you will play a vital role in coordinating care for patients and contributing to the hospital's mission to provide high-quality, patient-centered care.
Key Responsibilities- Conduct thorough history and physical examinations to inform patient care plans
- Develop and implement appropriate interventions and procedures in accordance with established standards of care
- Coordinate patient care with interdisciplinary teams to ensure seamless transitions and optimal outcomes
- Collaborate with physicians and other healthcare professionals to provide comprehensive care
- Participate in quality improvement initiatives to enhance patient care and outcomes
- Master's Degree in Nursing (Nurse Practitioner) or a college degree as a Physician's Assistant from a recognized university
- Preferred: One (1) year of experience in a similar role
