Hospice Care Coordinator

We're seeking a compassionate and skilled RN Case Manager to join our team at Remarkable Hospice Companies. As a key member of our interdisciplinary team, you'll play a vital role in delivering high-quality patient care and services to our in-home clients.
Responsibilities- Assess patient needs and provide hands-on care to ensure their comfort, safety, and well-being.
- Collaborate with the hospice physician and interdisciplinary team to develop and implement patient care plans.
- Communicate effectively with patients, families, and healthcare providers to ensure seamless care coordination.
- Document patient information accurately and thoroughly in our electronic healthcare record.
- Coordinate ancillary services, such as DME, lab, and transportation, to support patient care.
- Provide education and support to patients, families, and caregivers to promote informed decision-making and self-care.
- Current RN license in good standing.
- Minimum 1 year of nursing experience, with hospice experience preferred.
- Current CPR certification.
- Strong organizational and communication skills.
- Proof of current auto insurance.
- 15 paid days off plus 8 holidays.
