We are seeking a skilled and compassionate Social Worker to join our team at Remarkable Hospice Plano. As a key member of our care team, you will play a vital role in providing high-quality psychosocial support to our patients and their families.
Key Responsibilities- Assess the psychosocial status of patients and develop individualized plans of care to meet their unique needs.
- Provide short-term individual counseling, community resource planning, and crisis intervention to patients and their families.
- Collaborate with the healthcare team to ensure comprehensive care and coordination of services.
- Document patient progress and provide timely, accurate reports to ensure continuity of care.
- Participate in quality improvement initiatives and contribute to the development of policies and procedures.
- Master of Social Work (MSW) degree from a Council on Social Work Education-accredited program.
- State license to practice as a social worker.
- 12 months of experience in social work or a related field.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.
